I would say that started life as a round back oscillator.  The "shelf" that the oscillator link connects to is straight across while on the next (3-star) the link is connected to the bottom of the loop casting but at a higher level.  With the rear cover for a round back osc. you'll probably be able to put the fan back to original.  The problem is that the back cover was made of pot metal and round backs are uncommon.  Maybe a cover from a GE from 1915 through 1924 will fit, maybe not but those covers/ (rear plates) are made of cast iron and have the gearbox cast integral with the cover.
